Job Title:               Bilingual Intake Specialist- Johnson County
Department:          Family Services
Reports To:           Financial Counselor
FLSA Status:         Full Time, non-Exempt

SUMMARY The Intake Specialist will have front line, receptionist duties and well as intake duties for Emergency Assistance,  Health Navigation programs and others as needed.  This individual will be the first point of contact to all clients, volunteers and partners visiting or calling the Olathe office of El Centro, Inc. The intake Specialist needs to have a wide knowledge of services, agencies and general layout of Johnson County as well as possessing excellent people skills and communication in both English and Spanish.  Work hours will be Monday – Friday 8:00am – 5:00 pm
 
 
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  1. Implement the Intake procedures for Emergency Assistance and Health Navigation Program at El Centro Inc. in Johnson County
  2. Keep and maintain service resource contact information for other agencies and social service providers for residents of Johnson County.
  3. Create appointments for Health Navigator in Johnson Country
  4. Maintain and Distribute specific assistance funds for clients that seek and qualify for emergency assistance with utilities, rental or other emergency situations.
  5. Implement the follow-up component of the Health Navigation Program.
  6. Record and track client and follow-up information in MAACLink Data Base
  7. Work closely with other Case Manager from other El Centro Departments.
  8. Carry out the mission, vision and strategies of El Centro, Inc.
  9. Other duties essential to role-modeling inherent in the position of a senior manager at El Centro, Inc.
 
QUALIFICATIONS 
 
  1. Demonstrated commitment to teamwork philosophy and willingness to function as a vital component of an overall team for the benefit of the community is required of everyone at El Centro, Inc.
  2. Bilingual English/Spanish written, and verbal communication skills commensurate with position are essential.
  3. Excellent Organizational Skills.
  4. Computer knowledge.
  5. Ability to identify and respond to the social, economic and cultural experiences
  6. Ability to tolerate and thrive in sometimes high-stress work environment
 EDUCATION and/or EXPERIENCE 

High School graduate, Associates degree and customer service experience
 
PHYSICAL DEMANDS
Driving ability preferred.  Reasonable accommodations will be made to enable individuals with disabilities to perform the essential functions.
 
WORK ENVIRONMENT
Ability to function in an indoor office environment required.  Some driving may be necessary.    Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

In late 2012 we learned that Mary Lou Jaramillo would retire by the end of 2013.  

We at El Centro want to honor Mary Lou in a special way.  Within the many educational, social and economic programs we offer, she had a special passion for our Academy for Children (AFC).  This fully accredited dual language (Spanish and English) preschool provides an excellent foundation for learning and growth.  The AFC has provided educational excellence for over 20 years.  Our children enter USD 500 (KC KS Public Schools) Turner School District, Catholic Schools and others making a positive impact and ready to learn.

El Centro requires each parent to make a payment for each child within a sliding scale depending on income.  In virtually all instances, El Centro is subsidizing these children’s’ tuition.  We need additional resources to keep our school the vibrant force in the community.

With this need El Centro has created the Mary Lou Jaramillo Scholarship Fund.   Gifts will be the critical element in allowing our most vulnerable children to attend the AFC and receive El Cento’s strong life changing foundation for future success.  Please help El Centro’s children by honoring Mary Lou with a financial gift!

Job Title:               Bilingual Health Navigator: Johnson County

SUMMARY The Health Navigator will guide the underserved and underinsured population of Johnson County through the complexities of the healthcare system through education, access and networking. The major role of the Health Navigator is to create access to healthcare for underserved population through an inside out approach. The Health Navigator's major responsibility is to develop strong community partnerships with Safety net clinics, local Hospitals, Health Departments, low-cost health insurance providers; be aware of their policies; and have personal contacts within these organizations. The Health Navigator will assess the nature of client’s health risks and assess the significant factors that serve as barriers to clients receiving treatment and working on a plan to eliminate those barriers with the ultimate goal of finding them a medical home or quality health care to meet their needs.
 
 
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  1. Implement the Health Navigation Program at El Centro Inc. in Johnson County
  2. Assist in creating access to healthcare for underserved populations of Johnson County

  4. Develop a patient navigation plan that addressees the client’s barriers with the most significant goal of establishing a medical home.
  5. Implement the follow-up component of the Health Navigation Program.

  7. Comply with grant requirements.
  8. Submit grant reports on time.
  9. Work closely with other health navigators/ Case Managers from other El Centro Departments.
  10. Carry out the mission, vision and strategies of El Centro, Inc.
  11. Other duties essential to role-modeling inherent in the position of a senior manager at El Centro, Inc.
